Hawaii’s first Alzheimer’s prevention trial is now accepting patients. Hawaii Pacific Neuroscience has been selected as the Hawaii site for PrevenTRON, a global Phase 3 study testing whether trontinemab can delay or prevent Alzheimer’s disease in people who have no memory symptoms yet but are at high risk.

This is a meaningful shift in approach. Rather than treating Alzheimer’s after symptoms appear, the study asks whether intervening earlier, before cognitive decline begins, can change the course of the disease.

What Makes Trontinemab Different

Trontinemab is a bispecific 2+1 BrainShuttle™ monoclonal antibody, and a reformulation of gantenerumab. It is engineered to use transferrin receptor 1-mediated transport to cross the blood-brain barrier, which allows efficient delivery into the central nervous system at substantially lower systemic doses than conventional anti-amyloid antibodies.

In practical terms, more of the drug reaches the brain and less circulates through the rest of the body.

Encouraging Early-Phase Results

The earlier BRAINSHUTTLE AD Phase 1b/2a study, which Hawaii took part in, reported rapid amyloid clearance along with early shifts in tau and neurogranin biomarkers.

91%

reached amyloid levels below 24 centiloids at 28 weeks

72%

reached levels below 11 centiloids

A Lower Rate of ARIA-E

Amyloid-related imaging abnormalities with edema (ARIA-E) are a known safety concern with anti-amyloid therapies. In the 1.8 and 3.6 mg/kg dose cohorts combined, ARIA-E continued to be observed in fewer than 5% of participants (4 of 149), which is substantially lower than the rates reported for approved anti-amyloid antibodies.

Reported ARIA-E rates

Trontinemab: under 5%

Lecanemab: 13%

Donanemab: 24%

About the PrevenTRON Study

PrevenTRON is a randomized, multicenter, double-blind, placebo-controlled Phase 3 study. It aims to enroll approximately 1,600 cognitively unimpaired participants who are at high risk of progressing to symptomatic Alzheimer’s disease.

Who Qualifies?

✓ Age 55 to 80

✓ MMSE score above 25 (no memory symptoms)

✓ A family history of Alzheimer’s, or a positive result for PTau217 or ApoE4

Final eligibility is determined by the study team after a screening evaluation.
